Home Visits are performed by the program coordinator to provide emotional support and assess ongoing needs of clients.
MWIN provides case management for MWIN clients to confirm continued eligibility for MWIN services, monitors their risk factors and offers a regular source of emotional support.
Example: Following assessment during a home visit of a Navy Survivor who was unable to prepare meals independently, MWIN established and paid for Meals-on-Wheels service to assure healthier food choices and hot meals.
